These forms help Nixiwaka understand your background, health, and intentions before ceremony — and track how the work lands in your life in the months and years after.
All surveys are confidential and reviewed personally by Nixiwaka before every ceremony. They are not a screening tool to exclude people — they are a care tool to include them more fully.
Take your time. Answer as honestly as you can. There are no right or wrong answers. The more Nixiwaka knows about where you are coming from, the better he can serve you in the ceremony space.
Certain health conditions and medications require specific care or preparation in ceremony. The survey allows Nixiwaka to be fully informed before you arrive.
Understanding what you are bringing — your history, your questions, your wounds — allows the facilitators to hold you more precisely in the ceremony space.
Every participant affects the field of a ceremony. Knowing the group helps Nixiwaka create the right container for everyone present.
Both forms below must be submitted at least 48 hours before your ceremony date.
Not required, but deeply valued. Your reflections — at different distances from the ceremony — help Nixiwaka understand what is working and serve future participants better.
